RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT ("AGREEMENT")

In consideration of participating with Valley Leagues I represent that I understand the nature of this Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the Activity.

I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost and damages incurred as a result of my participation in the Activity.

I hereby give Valley Leagues, its assigns, licensees, and legal representatives the irrevocable right to use my name, picture, video, portrait or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any rights to inspect or approve the finished product, including written copy, which may be created in connection therewith. I hereby release, discharge, and covenant not to sue Valley Leagues, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lesser of premises on which the Activity takes place, (each considered on of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.

I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

By submitting this online form I agree to all Valley League Rules and Regulations including the Refund and Returns policies that are located at Valleyleagues.com. I am aware and agree that a profile must be taken to participate in the organization.

COVID RULES

As a participant of our League, you agree to the following rules and are now considered as USER.

CV1. There is a risk of transmission of COVID-19 in crowded places or people in close proximity to one another and the User acknowledges its use will cause people to gather. User assumes all risks, known and unknown, arising from its use of the School and COVID-19 and agrees such assumption shall be broadly interpreted in favour of our organization and or the location/gym. User shall be responsible and liable for any costs, claims, lawsuits and damages arising directly or indirectly from the use, the invitees and COVID-19 and shall indemnify our organization and or the location/gym.

In the event of any cancellation or termination of use, no refunds shall be provided.

Our organization and or the location/gym does not represent or warrant its properties are safe to use while COVID-19 has not been eradicated or controlled by a vaccine.

CV2. Federal, State, County, and City Safety Precautions. User, at his/her/its sole expense, shall observe and comply with ALL applicable requirements and guidelines for COVID-19 for its use and shall cause any invitee to comply with such requirements and guidelines.

The term "invitee" shall mean any participant, spectator, member, employee, volunteer, or any person in the use area due to the User’s use (such as, but not limited to, umpire or referee).

CV3. Our organization and or the location/gym Safety Precautions. User shall require each invitee to comply with:

     (i)   the use of a cloth face mask or other face-covering;

     (ii)   a temperature scan prior to entering the area of use, and any invitee with a temperature in excess of 98.6-degrees shall be denied entrance;

Our organization and or the location/gym may adjust or impose new safety precautions upon notice to the User.

The applicable requirements and guidelines for COVID-19 in Section CV2 and the above our organization and or the location/gym requirements shall be collectively referred to as "COVID Requirements."

CV4. Compliance.

     (i)   If the User cannot comply with COVID Requirements before, during and after the use, User shall cancel the use.

     (ii)   During its use if User notices non-compliance with COVID Requirements, User shall stop its use immediately and cause the non-complying invitee(s) to vacate the School immediately; provided, that if User believes it cannot comply with COVID Requirements, User shall stop its use immediately and cause all invitees to vacate the School. User shall notify Los Angeles School Police and/or local law enforcement if any invitee refuses to vacate;

     (iii)   Notwithstanding any provision, our organization and or the location/gym may terminate User’s use or User’s Civic Center Permit upon delivery of notice if our organization and or the location/gym, in its sole and absolute discretion, believes User cannot comply or is not complying with COVID Requirements; and

     (iv)   Notwithstanding any provision, LAUSD, in its sole and absolute discretion, may decline to allow User any future use of our organization and or the location/gym properties based on User’s past conduct and/or compliance with COVID Requirements.

FREE AGENTS

I am aware that this online form does not guarantee me a position or right to participate in the league. I am aware that I will be added to a Free Agents Pool and as teams need players, the captains will contact the commissioners of the league and request my information. My first and last name will appear on a public list for all to see but my personal contact information (Email, Phone) will not be visible. I am aware that all other information (Height, Weight, BOD, ect..) will be posted for the captains to select what is best for their team.
